Payment & Order Details:

• Checks: Payable to “McMillen Dazzler Booster Club” – accepted through May 18 (contact Kayla via text at 760-529-2994 if not paying at the sale)

• Credit Cards/Online: current page/order form - May 13–18

• Order Deadline: Submit completed forms and payment by Sunday, May 18. Forms must be printed or scanned—no photos accepted.
